Some patients experience forefoot problems such as metatarsalgia, interdigital neuritis, or neuroma. While some of these problems can be controlled with the use of foot devices, sometimes the metatarsal area needs additional elevation to provide additional structural support.

The metatarsal advancement is placed above the device and its anterior lip fits snugly over the end of the orthosis. It is typically placed in the center of the device to elevate the three medial metatarsals, but it may be moved slightly laterally or medially to the midline of the device to provide support.
